Application format and required materials

CONTRIBUTIONS REQUESTED BY MEMBERS FOR A SPECIFIC ORGANIZATION ARE BROUGHT TO THE BOARD AND ARE VOTED ON DURING A QUARTERLY MEETING OR VIA CONFERENCE EMAIL. MEMBER REQUESTS MUST HAVE A SET AMOUNT FOR CONTRIBUTION AND A DESCRIPTION OF THE ORGANIZATION AND ITS MISSION. FOR INDIVIDUALS REQUESTING FINANCIAL ASSISTANCE, THE WRITTEN APPLICATION MUST INCLUDE REASON FOR NEEDING ASSISTANCE AND PROOF OF HARDSHIP. THESE APPLICATIONS ARE BROUGHT TO THE BOARD AND ARE VOTED ON DURING A MEETING OR EMAIL EXCHANGE.

Any restrictions or limitations on awards, such as by geographical areas, charitable fields, or kinds of institutions

CONTRIBUTIONS TO ORGANIZATIONS ARE LIMITED TO $1,500 PER CALENDAR YEAR. FINANCIAL ASSISTANCE CONTRIBUTIONS ARE LIMITED TO $500.00 PER MONTH FOR A MAXIMUM OF THREE MONTHS IN ONE CALENDAR YEAR. THESE AMOUNTS MAY BE INCREASED ON A PER ORGANIZATION/INDIVIDUAL REQUEST AFTER REVIEW BY THE BOARD.
