IN-PATIENT DEPARTMENTS (MED/SURG AND SWING REHAB) & AMBULATORY SURGERY (OR): IN 2020, THE HOSPITAL AND THE FOUNDATION SUPPORTED OUR STAFF WITH TUITION ASSISTANCE AND REIMBURSEMENT FOR EDUCATION RELATED TO HEALTH CARE SERVICES. THIS GENEROUS ASSISTANCE MADE IT POSSIBLE FOR SEVEN OF OUR STAFF MEMBERS TO PURSUE HIGHER DEGREES IN HEALTH CARE RELATED FIELDS. ERH TAKES PRIDE IN SERVING AS A TEACHING HOSPITAL AND HOSTING STUDENTS FROM MANY SCHOOLS SUCH AS SUNY SULLIVAN, SUNY ULSTER, SUNY ORANGE, BOCES NEW VISIONS PROGRAM, LEMOYNE COLLEGE, TOURO MEDICAL COLLEGE, CLARKSON UNIVERSITY, UNIVERSITY AT BUFFALO, ALBANY COLLEGE OF PHARMACY, RHODE ISLAND UNIVERSITY COLLEGE OF PHARMACY, ANTIGUA MEDICAL UNIVERSITY AND MARIA COLLEGE, PACE UNIVERSITY, SACRED HEART UNIVERSITY, EASTWICK COLLEGE, LONG ISLAND UNIVERSITY, SUNY EMPIRE, ADELPHI UNIVERSITY, RUSSEL SAGE COLLEGE, MARIST COLLEGE, STONYBROOK UNIVERSITY, BAY PATH UNIVERSITY. IN 2020, THE COVID PANDEMIC PRESENTED A MONUMENTAL CHALLENGE FOR MANY HEALTHCARE STUDENTS; NEVERTHELESS, OUR HOSPITAL FOUND WAYS TO ASSIST SCHOOLS BY PROVIDING CLINICAL ROTATIONS. IN 2020, STUDENTS FROM TWO LOCAL COMMUNITY COLLEGES BENEFITED FROM WORKING SIDE BY SIDE OUR SKILLED AND COMPASSIONATE CLINICAL TEAM. DURING THEIR CLINICAL ROTATIONS, THEY OBSERVED FIRSTHAND THE DELIVERY OF EXCEPTIONAL CARE IN BOTH OUR INPATIENT UNIT AND EMERGENCY DEPARTMENT. AS IN PAST YEARS, ERH PROVIDED THESE SERVICES WITHOUT REQUIRING ANY MONETARY COMPENSATION. PATIENT SATISFACTION AND POSITIVE OUTCOMES REMAIN SIGNIFICANT QUALITY INDICATORS. IN 2020, PATIENT SATISFACTION SCORES REACHED 99% FOR THE INPATIENT SERVICES. OUR NURSING HOME PLACEMENT RATE WAS AN EXTREMELY LOW 3.7%. THESE STELLAR RESULTS ARE THE PRODUCT OF A MULTIDISCIPLINARY COLLABORATIVE EFFORT AMONG NURSING, PHYSICAL THERAPY, OCCUPATIONAL THERAPY, SPEECH-LANGUAGE PATHOLOGY, SOCIALIZATION ACTIVITIES, AND A REGISTERED DIETITIAN. BY COMBINING QUALITY CARE, EVIDENCE-BASED RESEARCH, EXCELLENT NURSING SKILLS, AND EXPANDING OUR CLINICAL EDUCATION PROGRAMS, THE IN PATIENT UNIT AT ELLENVILLE REGIONAL HOSPITAL MADE IT OUR GOAL TO IMPROVE PATIENT OUTCOMES ACROSS THE CARE SPECTRUM. WHILE THE COVID 19 PANDEMIC ROCKED THE CORE OF OUR HEALTHCARE SYSTEMS, OUR ORGANIZATION REMAINED A STRONG AND RESILIENT HEALTH CARE FACILITY. IN THE MIDST OF THE COVID 19 DEVASTATION, OUR ERH TEAM CAME TOGETHER DEVELOPING AND IMPLEMENTING INNOVATIVE MITIGATION STRATEGIES SUCH AS CREATING NEGATIVE PRESSURE ROOMS, SECURING MUCH NEEDED MEDICAL DEVICES, SUPPLIES AND PPE (PROTECTIVE PERSONAL EQUIPMENT). AS A RESULT, OUR FACILITY WAS ABLE TO HOLD STEADFAST COLLABORATING WITH OTHER AGENCIES AND PROVIDING QUALITY, COMPASSIONATE CARE AND GENERATING POSITIVE OUTCOMES. IN 2020, WE SERVED PATIENTS IN NEED OF MEDICAL MANAGEMENT DURING DRUG AND ALCOHOL WITHDRAWAL PRIOR TO THEIR TRANSITIONING TO EITHER AN INPATIENT OR OUTPATIENT REHAB PROGRAM. WE HAVE WORKED VERY HARD TO HAVE PROTOCOLS IN PLACE TO FACILITATE A SEAMLESS TRANSITION FOR THIS POPULATION OF PATIENTS. IN 2020, SAFETY CONTINUED TO BE A PARAMOUNT IN OUR FACILITY. WE TRACKED AND REPORTED COMPLIANCE WITH MEDICATION SCANNING. THIS PRACTICE ENHANCES SAFE MEDICATION ADMINISTRATION BY HELPING VERIFY PATIENT IDENTIFICATION AND CORRECT MEDICATION AND SUBSEQUENTLY ENSURING CORRECT MEDICATION ADMINISTRATION AND MITIGATING POTENTIAL ERRORS. WE WERE ABLE TO SIGNIFICANTLY BOOST OUR SCORES AND COMPLIANCE LAST YEAR. IN 2020, OUR SAFE PATIENT HANDLING COMMITTEE REVIEWED AND MAINTAINED SUCCESSFUL PRACTICES THAT MAXIMIZED A SAFE ENVIRONMENT FOR OUR PATIENTS AND STAFF. THIS INITIATIVE WAS DRIVEN BY A MULTIDISCIPLINARY TEAM COMPRISED OF MEMBERS OF OUR NURSING, REHABILITATION, RADIOLOGY, QUALITY, AND FACILITIES STAFF FOCUSED ON THIS INITIATIVE TO ENSURE THAT OUR STAFF ARE INJURY FREE. THE COMMITTEE HAS MADE A TREMENDOUS EFFORT TO DELIVER GUIDANCE AND TRAINING STAFF ON TRANSFERS, BODY MECHANICS, AND USE OF ASSISTIVE DEVICES. THEY HAVE DONE THIS VERY EFFECTIVELY BY INCORPORATING EVIDENCE-BASED PRACTICES IN OUR EVERYDAY TASKS AND ROUTINES LIKE THE BMAT MODEL (BED SIDE MOBILITY ASSESSMENT TOOL). THIS TOOL ASSISTS WITH ASSESSING PATIENT'S CAPACITY TO MOVE AND COORDINATION STATUS MAKING TRANSFERS SAFER. THE SAFE PATIENT HANDLING COMMITTEE CONTINUES TO MEET REGULARLY TO DISCUSS EDUCATIONAL NEEDS AND PROVIDE HANDS-ON TRAINING TO ALL CLINICAL STAFF. WE WERE ABLE TO SECURE NEW BEDS WITH IN BED ALARMS AND SPECIALTY MATTRESSES TO ASSURE SAFETY AND MAINTAIN SKIN INTEGRITY. STAFF EDUCATION IS ALSO AN ESSENTIAL ELEMENT IN MAINTAINING AND PROMOTING CLINICAL COMPETENCY. ONE HUNDRED PERCENT OF OUR REGISTERED NURSES MAINTAIN CERTIFICATION IN BASIC AND ADVANCED LIFE SUPPORT THROUGH THE AMERICAN HEART ASSOCIATION AND TELEMETRY. OUR NURSE EDUCATOR PROVIDES EDUCATION OPPORTUNITIES VIA IN-SERVICES, WEBINARS, LECTURES, AND SCHOLARLY ARTICLES. THE HANDS ON TRAINING AND ACADEMIC OFFERINGS ENSURE OUR CLINICAL STAFF IS KNOWLEDGEABLE ON CURRENT TRENDS AND STANDARDS OF CARE. WE LINK ALL NEW HIRES WITH AN EXPERIENCED PRECEPTOR AND ADJUST THE ORIENTATION PROCESS TO MEET THE NEEDS OF EACH INDIVIDUAL. IN OUR FACILITY, EDUCATION DELIVERY INVOLVES EVERYONE. THE NURSE EDUCATOR COLLABORATES WITH THE NURSING LEADERSHIP TEAM, DIRECTOR OF PHARMACY, DIRECTOR OF REHABILITATION SERVICES, CLINICAL IT APPLICATIONS SPECIALIST, AND THE MANAGER OF RESPIRATORY SERVICES TO OFFER COMPREHENSIVE ANNUAL EDUCATION FOR ALL NURSING STAFF. HEALTH AND HUMAN SERVICES ISSUED PROVIDER RELIEF FUNDS, WELL NEEDED STIMULUS MONEY FOR COVID PREPAREDNESS. SOME OF THOSE FUNDS WERE USED TO PURCHASE ADDITIONAL PATIENT BEDS ENHANCING OUR SURGE CAPACITY FOR PATIENT CENSUS DUE TO COVID. THESE BEDS WERE EQUIPPED WITH SPECIAL FEATURES FOR CARE OF COVID PATIENTS SUCH AS, BUT NOT LIMITED TO SPECIALTY MATTRESSES TO PREVENT SKIN BREAKDOWN, BUILT IN SCALES FOR WEIGHT MONITORING, AND EXIT ALARMS TO REDUCE RISK OF PATIENT FALLS. EMERGENCY DEPARTMENT:ELLENVILLE REGIONAL HOSPITAL EMERGENCY DEPARTMENT IS A VITAL SERVICE FOR OUR COMMUNITY. A RURAL SETTING CAN MAKE IT DIFFICULT FOR RESIDENTS TO ACCESS HIGH-QUALITY EMERGENCY CARE. BY COMBINING QUALITY CARE, EVIDENCE-BASED RESEARCH, EXCELLENT NURSING SKILLS, AND EXPANDING OUR CLINICAL EDUCATION PROGRAMS, THE EMERGENCY DEPARTMENT AT ELLENVILLE REGIONAL HOSPITAL MADE IT OUR GOAL TO IMPROVE PATIENT OUTCOMES ACROSS THE CARE SPECTRUM. WHILE THE COVID 19 PANDEMIC ROCKED THE CORE OF OUR HEALTHCARE SYSTEMS, OUR ORGANIZATION REMAINED A STRONG AND RESILIENT HEALTH CARE FACILITY. WE CONTINUED TO PROVIDED EXCELLENT CARE IN A MORE EXPEDIENT MANNER THAN OTHER EMERGENCY DEPARTMENTS ACROSS THE COUNTRY. OUR 2020 AVERAGE FOR ARRIVAL TO TRIAGE WAS A STELLAR SEVEN MINUTES. ADDITIONALLY, THE AVERAGE LENGTH OF STAY WAS 95 MINUTES. THESE REMARKABLE TURNAROUND TIMES DERIVE FROM THE DEDICATION OF OUR MULTIDISCIPLINARY TEAM COMPRISED OF ED PROVIDERS, NURSES, PATIENT CARE TECHNICIANS, REGISTRATION, LAB, AND RADIOLOGY. THE TEAM'S PRIMARY FOCUS IS OPTIMAL PATIENT CARE. OUR PATIENT'S NEEDS AND OUR COMMUNITY ALWAYS REMAIN OUR TOP PRIORITY. IMPROVING PATIENT OUTCOMES IS THE PRIMARY DRIVER IN OUR EVERYDAY OPERATIONS. WHILE MANAGING THE EFFECTS OF THE COVID PANDEMIC WE CONTINUE OUR FOCUS ON THE OPIOID CRISIS. OUR EMERGENCY DEPARTMENT CONTINUED TO FINE-TUNE AND FURTHER DEVELOP THE PROJECT RESCUE INITIATIVE. THE ED PROVIDERS ARE CERTIFIED TO PROVIDE MAT (MEDICATION ASSISTED TREATMENT). THIS HAS GREATLY FACILITATED TREATMENT OF PATIENTS PRESENTING TO THE ED AFTER AN OVERDOSE. WE HAVE CONTINUED OUR COLLABORATION EFFORTS WITH THE ULSTER COUNTY SHERIFF'S DEPARTMENT IN THE ORACLE PROGRAM AND UTILIZE THE SERVICES OF OUR IN-HOUSE SUBSTANCE USE DISORDER (SUD) PROGRAM. OUR COLLABORATIVE EFFORTS HAVE HELPED WITH INDUCTION OF MAT AND SECURING INPATIENT AND OUTPATIENT REHABILITATION SERVICES FOR THOSE WHO ARE IN NEED. WE ALSO CONTINUED TO WORK CLOSELY WITH OUR LOCAL EMS AGENCIES. IN 2020, ERH PROVIDED OUR LOCAL EMS PROFESSIONALS WITH MEDICAL EDUCATION ACTIVITIES RELATED TO MANAGEMENT OF PATIENTS PRESENTING WITH COVID 19 SIGNS AND SYMPTOMS. WITH THE EVER-CHANGING UNCERTAINTIES ZOOM STAFF MEETINGS AND EDUCATION SESSIONS WERE HELD PERIODICALLY TO MAKE SURE ALL STAFF WERE UP-TO-DATE ON EMERGING ISSUES AND TREATMENT REQUIREMENTS. DURING THE HEIGHT OF THE PANDEMIC, THE ED NURSES AND ANCILLARY STAFF DEMONSTRATED TRUE TEAM SPIRIT, COURAGE AND UNWAVERING PERSEVERANCE. THEIR RESILIENCY WAS VITAL IN ESTABLISHING EFFECTIVE DRIVE-UP TESTING AND VACCINATION CLINICS HELD AT ERH. HEALTH AND HUMAN SERVICES ISSUED PROVIDER RELIEF FUNDS, WELL NEEDED STIMULUS MONEY FOR COVID PREPAREDNESS. SOME OF THOSE FUNDS WERE USED FOR PATIENT MONITORING EQUIPMENT, AND NURSING CARTS. RADIOLOGY:IN 2020, THE RADIOLOGY & CARDIOLOGY DEPARTMENTS AT ELLENVILLE REGIONAL HOSPITAL (ERH) HAD A STRONG START WITH ALMOST ALL OUTPATIENT MODALITIES PERFORMING ABOVE AVERAGE. AFTER THE PROMISING START, THE COVID 19 PANDEMIC HIT, WHICH SLOWED ALL OUTPATIENT SERVICES. MANY OF OUR STAFF WERE FURLOUGHED AND THE NEXT SEVERAL WEEKS WERE SPENT IN PREPARATION FOR OUR REOPENING OF OUTPATIENT SERVICES WITH THE NEW INFECTION CONTROL GUIDELINES. ALL STAFF WERE BROUGHT BACK AND OUTPATIENT SERVICES WERE OPENED BACK UP AT THE END OF MAY. OVERALL, RADIOLOGY AND CARDIOLOGY RETURNED TO NORMAL PATIENT VOLUMES WITH A MODEST INCREASE IN EXAMS. MAMMOGRAPHY HAD ITS ANNUAL MQSA INSPECTION COMPLETED WITH NO CITATIONS OR RECOMMENDATIONS AND OUR TECHNOLOGISTS WERE PROVIDED ADDITIONAL TECHNICAL TRAINING TO ENSURE OUR PATIENTS RECEIVED THE HIGHEST LEVEL OF CARE. WE REDUCED OUR X-RAY REPEAT/REJECT RATE BY 44% COMPARED TO THE PREVIOUS YEAR IN AN EFFORT TO LIMIT RADIATION EXPOSURE TO PATIENTS, WHICH ULTIMATELY LED TO IMPROVED IMAGE QUALITY.FOR NUCLEAR MEDICINE, ERH PURCHASED A TRACKING SOFTWARE SYSTEM, WHICH STREAMLINES RECORD KEEPING FOR COMPLIANCE AND IS CAPABLE OF TRACKING VALUABLE PATIENT INFORMATION AND STATISTICS FOR ALL ORDERING PROVIDERS.ERH CONTINUED TO EXPAND ON THE SERVICES PROVIDED BY THE QUALITY IMPROVEMENT GRANT, WHICH WAS AWARDED THE PREVIOUS YEAR. WITH THIS GRANT, ERH PERFORMED 189 CT CALCIUM SCORE EXAMS AS A FREE OF CHARGE SERVICE TO THE PATIENTS. OFFERING THE EXAMS AT NO OUT OF POCKET COST TO THE PATIENTS HAS BEEN SUCCESSFUL WITH MANY PATIENTS RECEIVING CRUCIAL FOLLOW-UP CARE BASED ON THE TEST RESULTS.
