GRANTEES ARE INVITED TO COMPLETE A GRANT APPLICATION ONLINE THROUGH AN INVITE-ONLY PORTAL. GRANTEES MUST SET UP AN ENTITY ACCOUNT TO VERIFY THEIR FEIN AND CHARITABLE STATUS. THE GRANT APPLICATION REQUIRES THE FOLLOWING: - PURPOSE OF THE GRANT REQUEST - TOTAL AMOUNT BEING REQUESTED OVER THE ENTIRE GRANT TERM - PROGRAM START AND END DATE - MISSION STATEMENT - ORGANIZATION STATEMENT DESCRIBING MAJOR GOALS, ACTIVITIES, ETC. - GEOGRAPHICAL AREA SERVED - NEED FOR GRANT AND THE PROBLEMS THE GRANT WILL ADDRESS - GRANT OUTCOMES OR RESULTS TO BE ACHIEVED - LIST OF CURRENT BOARD MEMBERS
