Holistic Case Management for the Homeless
Case Management: Provide holistic personal care to individuals who are homeless or formerly homeless. We focus on identifying housing, access to physical/mental healthcare, and building support by using various evidenced based practices. In 2022, our Homeless Prevention project aided in allowing 26 households (57 unique individuals) in retaining housing. We assisted 24 individuals to exit homelessness to positive destinations (accessing emergency shelter, reconnecting with friends/family members, etc.). Our ReadyID program helped 205 individuals acquire important documents such as state IDs, birth certificates, and social security cards, which are required for gaining housing or jobs, another critical step towards self-sufficiency.Open Table Ministry operated both a congregate and non-congregate shelter in 2022. Our non-congregate hotel shelter program provided more than 12,000 nights of emergency shelter to 363 of the most vulnerable unsheltered families and individuals in the Durham community. Free Store: Provide our homeless clients with basic needs such as new clothing, shoes, hygiene items, and sleeping bags to keep sheltered from the weather, and furniture/basic household goods for those transitioning from homelessness to permanent housing. In 2022 the Free Store served 843 individuals and distributed 20,197 items including 449 winter coats, 1,307 hygiene kits, 737 pairs of shoes, 970 pairs of jeans, 3,438 shirts, and 406 backpacks. Get To Work: We provide work-related items such as work boots, nonslip shoes, and uniform components to individuals who have job opportunities for which such items are required, thus enabling them to start work immediately.
