Avow Care Services provides outpatient palliative consults from referring physicians caring for patients with acute or chronic illnesses that may not necessarily be terminal. Symptomatic and supportive disease management and counseling is provided to this patient population until they qualify and choose to transition to hospice care or choose to remain under palliative care with continued disease modifying treatments. During 2023, the organization provided 2,907 care and counseling visits to patients and their families.
