Community Grief Support Services
For the year ending June 30, 2021, Pathways served 724 caregiver clients and 27 hospice clients/families as well as 11 family bereavement follow-ups. Our widow's support group provided support for 29 people from the community and Journey's, our adult bereavement program, provided support for 26 participants. Pathways family-centered community grief education and counseling program (CHANGES) had 134 participants and 63 students participated in our school-based on-campus bereavement groups through Zoom (app used to interact with a group remotely using the internet). In addition, Pathways provided grief and loss presentations to 545 students, 177 parents, and 629 staff throughout our local school community, also through Zoom platform.In response to the first 2 1/2 months of the Covid-19 Stay-at-Home order, Pathways delivered 310 Essential Care packages and 180 hot meals to the most isolated and at-risk clients before any other food security and PPE programs was available through city and county programs. In December 2017 Pathways launched a new Care Navigation Program initiative in collaboration with Meals on Wheels Long Beach and the California State University Long Beach nursing department. The Pathways Care Navigation Program is a model of care designed to help vulnerable home-bound older adults live safely and as independently as possible by providing access to appropriate healthcare and community services.
