Enhanced Care Management Services
In 2024, Inland Housing Solutions focused on growth, innovation, and delivering high quality services. Launching the Enhanced Care Management program in July 2024 has given IHS the opportunity to provide clients with a greater variety of direct services, specifically mobile-based healthcare and behavioral health services. IHS continued to serve an expanded target population, including individual households, many of which are at-risk of homelessness or homeless, and medically fragile, elderly, and/or disabled, in both Riverside and San Bernardino Counties. Across all programs, Inland Housing Solutions served 2,749 households, providing housing, housing navigation, move-in assistance, case management and more. Program highlights include:Rapid Re-housing: Housing Navigation, permanent housing, move-in and rental assistance, and intensive case management for 30 families with children.Permanent Supportive Housing: In partnership with Inland Empire Health Plan (IEHP), rental subsidy, tenancy support and supportive services for 166 formerly chronically homeless households.Community Supports: In partnership with IEHP, a variety of non-medical, medically appropriate services for 1,739 at-risk or homeless households, including housing navigation, tenancy support, home modifications, and housing deposits. Housing Navigation Housing Choice Voucher (program sunset in January 2024): In partnership with the Housing Authority of the County of San Bernardino, housing navigation, application fees and security deposit assistance for 11 HACSB voucher holders.Housing Navigation Adult Protective Services (program sunset in November 2024): In partnership with the San Bernardino County Department of Aging and Adult Services, housing navigation and case management for 42 at-risk or homeless elder or dependent adults. Enhanced Care Management: In partnership with Inland Empire Health Plan (IEHP), comprehensive health assessment, care coordination, and direct mobile-based healthcare services for 761 Medi-Cal members with complex medical and social needs. Across programs, IHS housed 255 households. In 2024, IHS continued to coordinate the multi-agency County-Wide Housing Search effort. IHS partners with private sector property owners to identify suitable housing options for homeless individuals and families linked to rental subsidy and community-based supportive services. Property Owners play a vital role in our ability to provide rental housing opportunities for our most vulnerable neighbors and are a crucial partner in the mission to end homelessness in our communities. IHS works alongside multiple service agency partners through the County's 211 Coordinated Entry System (CES) to facilitate housing identification and placement services, while maintaining positive relationships with rental property owners and their operators.
