  • Community Health Engagement Initiative

    In 2019, AVOC, home of the Greater Cleveland Community Health Workers Association (GCCHWA), increased its capacity to serve as the community engagement catalyst for Cuyahoga Countys community health initiatives housed under the HIP-Cuyahoga umbrella, known as REACH. AVOC not only provides support as the connector of the clinic to community linkages strategy but the core programs of AVOC braided with Asset Based Community Development (ABCD) to work collaboratively with the all functions of the initiative helping to develop and shape a comprehensive community engagement framework

  • Community Health Hubs Development

    Presently, A Vision of Change has designed a Public Health Model of building Community Health Hubs utilizing CSU Community Health Workers and CSU student nurses as boots on the ground. With the additional support of the Cuyahoga County Board of Health, Fairhill Partners and the Prevention Research Center for Health Neighborhoods at Case Western Reserve University, the intent is to expand on our organizing skills, outreach/recruitment and promotion/dissemination strategies by building a pipeline of opportunities for residents/CHWs across the spectrum of achieving health equity and promoting health care efforts.
