Application format and required materials

The hyperlink to the MN Common Grant Application Form is available on the Foundation website, along with detailed grant application instructions. A cover letter from the requestor, stating the amount requested and the details of the specific purpose for which the grant funds will be used, is required. Also included: a brochure/explanation of the organization's work/mission, a W-9 form, and a commitment to use the grant funds within one of the Association's jurisdictional counties.

Submission deadlines
Must be submitted by the first Tuesday in May for each calendar year.
