Form is provided by Foundation,completed by applicant. The applicant must attach a list of officers and directors of the Organization,income and expense budget for the coming fiscal year, most recent Financial Statement,and statement of federal exemption. The applicant must provide a brief history of the mission and major program of the organization and 3 to 5 outcomes achieved during past year.

Grants are restricted to nonprofit organizations with priority given to the Greater Waukesha Area.
