All application must be in writing and must include the following information and materials:1. Applicant's name, address and telephone number.2. Name, address, and telephone number of the school you are attending.3. A copy of transcript from your school.4. Field of study.5. Expected graduation date.6. Evidence of your income (copy of W-2 form and tax returns for prior year) or if you are under 18 years old (copies of your parent's W-2 forms and tax returns for the prior year).6. Grade Point Average.

Awards are restricted to state of Alabama residents who are considered indigent, as defined by the most recently available federal guidelines.
