CHI OC deploys enrollment coordinators to inform, educate and empower residents through our community health access program. Covering outreach, enrollment and case management, implementation includes the following steps:Outreach: Coordinators conduct outreach and health insurance literacy efforts in hard-to-reach, low-income and minority communities, offering information in multiple languages.Enrollment: Clients are assisted with entering their information in a database to determine eligibility for 18 health and social service programs.
