There is no specific form of application. However a written request should be submitted to the Hedden Charitable Trust. The request should provide specific information which describes the dollar amount and the purpose of financial assistance requested and should include but not be limited to the benefit it will provide the applicant and how the funds will be accounted for. Current financial information of the applicant should be included with the request.
