Health Outreach Training
From July 1, 2022, to June 30, 2023, Health Outreach Partners conducted 11 webinar sessions with over 1183 participants; over 30% are documented to be from community health centers or primary care associations. Health Outreach Partners continued to collaborate with more than 18 National Training and Technical Assistance Partners (NTTAP) throughout the year on webinars, conference workshops, trainings, and learning collaboratives. Participant numbers were collected in some events, but health center status was not. Health Outreach Partners returned to a full travel schedule this year, with several in-person conferences and training workshops. These in-person events are back to pre-pandemic numbers. HOP led and participated in 8 total conference workshops and 6 training sessions, 2 in person and 4 virtual. During 2023, we led in-person workshops at the Western Forum for Migrant and Community Health, the Midwest Stream Forum, the East Coast Stream Forum, the National Conference for Agricultural Worker Health, the Northwest Regional Primary Care Association Spring Summit, and the California Primary Care Associations California Social Health Network Panel on Cross-Cultural Collaborations. We did not present any virtual conference workshops this fiscal year. We had two in-person trainings, one on Motivational Interviewing and Difficult Conversations at the Blue Ridge Medical Center in Arrington, Virginia, and an Outreach Worker training in Greenville, South Carolina for the South Carolina Primary Care Association. In our various trainings and workshops, we had over 443 participants, including health centers, community-based organizations, education, and leadership staff (including Board members). From February to July 2023, Health Outreach Partners (HOP) collaborated with Special Olympics, Inc. (SOI) to provide technical assistance in finalizing, distributing, and summarizing the Center for Inclusive Health Stakeholder Survey. The purpose of this survey is to help SOI develop tools and resources stakeholders would find most useful to help make healthcare more inclusive and accessible for those with intellectual disabilities. We collected 197 responses; over 51% reported being employed by a direct healthcare provider, and 22% reported being from community health centers. Also, during this period, HOP conducted 32 Learning Collaborative sessions, all web-based interactive peer learning training. Twenty-four of the sessions were in partnership with other NTTAP organizations, while HOP led 8 on our own. We reached a total of 236 unique individuals and 138 health centers during these sessions. HOP authored 10 total publications and one health education resource in this fiscal year for a total of more than 2170 downloads and page views combined. Among those publications were the TA articles addressing Diabetes Across the Lifespan, the SDOH screening Resource, Vol 3, Opioids- A Patient Resource, Sharing Their Stories: Documenting the True Impact of Transportation Barriers on Patient Health, and our new Organizational Self Care Assessment Tool. Additionally, the Special and Vulnerable Populations/Diabetes Task Force published a Summary Report of Resources based on activities from the last year. Those can be found at www.chcdiabetes.org. INFORMATION SERVICES & RESOURCES: From July 1, 2023, to June 30, 2024, Health Outreach Partners (HOP) conducted 25 webinar training sessions with approximately 3846 participants; over 57% are documented to be from community health centers or primary care associations. Health Outreach Partners collaborates with 20 National Training and Technical Assistance Partners (NTTAP) annually on webinars, conference workshops, trainings, and learning collaboratives. Participant numbers were collected for all our events, but health center status was not. Health Outreach Partners facilitated several in-person conference workshops, listening sessions, and trainings this year. HOP led and participated in 14 in-person events, 6 conference workshops, 3 training sessions, and 5 in-person listening sessions. These in-person conference workshops took place at the Western Forum for Migrant and Community Health, the Midwest Stream Forum, the East Coast Stream Forum, Community Health Institute, sponsored by the National Association of Community Health Centers, the Northwest Regional Primary Care Association Spring Summit, and the Idaho Community Health Center Associations Annual Outreach Conference. We did not present any virtual conference workshops this fiscal year. We had two in-person trainings, one on Trauma Informed Motivational Interviewing for Outreach Programs at the San Mateo County Department of Public Health, Healthcare for the Homeless/Farmworker Health Program and Outreach and Enrollment at Moses Lake Community Health Center, Quincey, Washington. In 2023, HOP committed to including the communities we serve in all program design, and the statement Nothing for us, without us representing our organizations values. Given this, we facilitated 5 in-person and 3 virtual listening sessions to inform future program design. The in-person events were held at the Western Forum for Migrant and Community Health, the Midwest Stream Forum, and the East Coast Stream Forum. In our various in-person events, we had over 355 participants, including health centers, community-based organizations, education, and leadership staff (including Board members). From April to July 2024, Health Outreach Partners (HOP) collaborated with HealthNet of Rock County, a free and charitable health center, to provide consultation and technical assistance in assessing financial and operational sustainability. HOP reviewed financial statements, available data, and staff interviews to complete our assessment and provide recommendations to HealthNet leadership for improved operations and funding diversification. Additionally, we offered a road map for applying for federally qualified health center status. Also, during this period, HOP led 12 Learning Collaborative sessions, all web-based interactive peer learning training sessions, which reached 183 unique individuals and 147 health centers. HOP authored four (4) publications and produced one health education podcast episode in this fiscal year for more than 1,213 downloads, and our resources, in total, received 42,546 page views combined. Among those publications were the Farmworker Health Network, Key Resource for Agricultural Worker Health, Outreach and Enrollment Training Curriculum for Health Center Staff and Outreach workers, the Organizational Self Care Assessment Tool Implementation Guide, and our Transportation is a Health Equity Issue fact sheet.
