Chenango Health Network Programs
The purpose of Chenango Health Network programming is to increase access to health related services and improve the population health of Chenango County through education, training, and environmental prevention strategies. The programs that CHN operated in 2022 include:Office of Rural Health. Funding from the State office of Rural Health allows our program and administrative staff to engage the community health promotion through; This funding also allows staff to participate in the Chenango County Housing coalition, Suicide Prevention Coalition, Older Americans group, Trauma Informed Care Regional Team, Chenango and Delaware County Behavioral Health Network, Head Start Health Advisory Counsel, Regional Health Workforce Development group, the Fetal Alcohol Syndrome Prevention regional group, and the Community Health Improvement Plan Team. Drug Free Chenango Community Coalition. This coalition is comprised of representatives from 12 different community sectors, coming together to increase the protective factors and reduce risk factors contributing to substance use disorder in Chenango County youth. Tobacco Free Zone. This tri county program administered by Cortland County Department of Health allows CHN to employ a Reality Check Coordinator to facilitate a youth group whose focus in on exposing Big Tobaccos strategies to obtain new smokers. The Reality Check Coordinator also provides community wide education on the benefits of smoke free housing, retail space and outdoor space. Navigator. The Navigator helps Chenango County residents enroll in the New York State Insurance Marketplace. CHNs navigators enrolled over 750 people in health insurance programs in 2022. Community Health Advocate. The Community Health Advocate helped over 540 Chenango County residents resolve post enrollment issues with their insurance. These issues included but are not limited to finding a primary doctor, getting pre approval for a medication or specialty appointment, billing disputes, financial assistance for cancer patents, prescription drug program enrollment, and support groups. Community Health Worker. The community health worker provided over 100 screenings for Substance Use Disorder and made over 200 separate referrals to treatment, Social Determinates of Health and recovery services in and around Chenango County in 2022. She also hosted 4 Drug take back events which provide safe disposal options for prescription medications. This position is part of a larger Rural Communities Opioid Response Program that covers 8 counties. Farm to School USDA. This is a collaboration with Norwich City School district. Chenango Health Network employs a Health Gardens Coordinator who works with an after school garden club, hosts local food taste tests in the cafeteria, and assist with the development of an Agricultural Curriculum. Small Wellness Program LGTBQ+. This funding allowed for the development of a training focused on health disparities in the LGTBQ+ rural community. The coordinator presented to 8 Health and Human service providers, covering the language, history, and strategies to combat this disparity.
