ALS Multidisciplinary Care Program
ALS Association staff working in the Greater Sacramento region, covering 24 counties in northern California, successfully met these needs:Served 302 patients with ALS, approximately 15 percent of which were veterans, along with 500 family members.Supported five regional ALS clinics, spanning more than 300 miles in distance to include those living in rural northern parts of the state. These clinics include UC Davis Multidisciplinary ALS Clinic, a Certified Treatment Center of Excellence; ALS/MS Neuromuscular RNCM, Sacramento VA Medical Center; ALS, Kaiser Permanente ALS Clinic Sacramento; Forbes Norris ALS Research and Treatment Center Satellite Clinic Sacramento; and Catalyst NeuroMedical ALS Multidisciplinary Clinic in Redding.Three care service managers provided 480 hours of staff time at the clinics, serving 233 patients with ALS at the ALS Multidisciplinary clinics.Provided 100 percent of newly diagnosed ALS patients with a care management assessment, comprehensive care plan, ongoing care management through routine outreach and supports to assess for changing needs, and a minimum of one care services engagement each quarter.Held 78 support group meetings for patients and caregivers, and 21 presentations and trainings on various medical, well-being and financial resource topics.Loaned out 358 pieces of equipment through the ALS Association's free equipment loan program.GeographiesNot indicatedDatesFeb 1, 2023 – Aug 31, 2023Source990No causes providedNo populations provided–$0
