A STATEMENT OF HOW THE GRANT WILL BE USED, 3. A STATEMENT OF THE BENEFITS TO BE DERIVED FROM SUCH A GRANT, 4. A STATEMENT OF NEED, AND 5. A STATEMENT VERIFYING THE APPLICANT'S 501 (C) STATUS. OTHER DOCUMENTS, SUCH AS A BRIEF HISTORY OF THE APPLICANT'S ORGANIZATION, STATISTICAL INFORMATION, ETC. MAY BE SUBMITTED WITH THE REQUEST.
