Information package that contians a brief background of the organization and its mission, list of board of directors, if applicable, amount of request, specific information about the purpose, need and use of the requested funds, appropriate budget information, copy of IRS exemption letter and previous year's audited financial statements if available, if not copy of most recent Form 990.
