Permanent Supportive Housing Case Management
Housing Case Management- The DRC provides Permanent Supportive Housing (PSH) Case Management services to individuals and families who have experienced chronic homelessness. To qualify for PSH assistance, an individual or household must have experienced 12 consecutive months of homelessness or four episodes of homelessness in the past three years and have an accompanying disabling condition as verified by a medical professional. DRC PSH Case Management gives these households access to a case manager who can assist them with lifes challenges. Whether its navigating the relationship with their landlord, locating special services such as mental or physical healthcare, attaining social security benefits, finding needed household items and furniture, or simply serving as an advocate for the household, PSH participants have a trained professional to rely on as support. Mobile Assessment/Street Outreach- The DRC Street Outreach team provides services to ensure citizens experiencing homelessness have access to housing opportunities and other resources in the community. DRC locates individuals and families experiencing unsheltered homelessness, completes assessments for housing, reunites them with family, if possible, helps them gather documentation required for housing program eligibility, and connects them with appropriate housing interventions within the Tarrant County Continuum of Care. The goal of the program is to assist unsheltered individuals and families towards housing in multiple ways. First, by ensuring that the household is enrolled in the local Continuum of Cares service system and regularly check in so that they can stay active and eligible to receive housing assistance. Second, by helping them to obtain any necessary critical documents needed for housing access. Critical Document Replacement- The DRC Critical Document Replacement team assists individuals and families experiencing homelessness in replacing needed critical documents such as: State ID, Drivers License, Social Security Card, Birth Certificate, and other documents often required for housing or employment opportunities. DRC has long standing relationships with issuing agencies in all 50 states that enables DRC to serve as advocates on behalf of the clients we serve.
