GRANT INQUIRIES SHOULD INCLUDE THE NAME OF THE ORGANIZATION, A CONTACT NAME WITH EMAIL AND PHONE INFORMATION, A REQUEST PURPOSE AND A REQUEST AMOUNT. THE FOUNDATION WILL ALSO REQUIRE FURTHER INFORMATION REGARDING THE ORGANIZATION, KEY LEADERSHIP, DETAILS ABOUT THE GRANT AMOUNT AND USE, IMPACT EXPECTED ON THE ORGANIZATION OR COMMUNITY, TIME LINE FOR THE REQUEST AND HOW THE RECIPIENT WILL MEASURE SUCCESS. THE FOUNDATION MAY ALSO REQUIRE A RECIPIENT ORGANIZATION TO PROVIDE: - CURRENT YEAR'S OPERATING BUDGET - LIST OF BOARD MEMBERS - EXPENSE AND INCOME BUDGET FOR THE PARTICULAR PROJECT OR PROGRAM - LIST OF BOARD MEMBERS - EXPENSE AND INCOME BUDGET FOR THE PARTICULAR PROJECT OR PROGRAM (IF APPLICABLE)
