Military Transition Support Program
Zero8Hundred provided Case Management Services to hundreds of local San Diego-based transitioning service members addressing mental health needs through a social work model. Zero8Hundred is staffed by a team of credentialed social workers and backed by an evidence-based veteran wellness model that has been addressing the needs of transitioning service members in the San Diego community for 9 years. The positive effect that social workers have during the transition process supports veterans throughout their veteran lifecycle and is the catalyst to a successful transition. Through its Military Transition Support and Suicide Prevention Program, Zero8Hundred serves 525 unique clients annually with over 8,500 social work service instances. Through this interaction, Zero8Hundred has served clients addressing mental health along with workforce development, basic needs, and education - all delivered through individual care plans.
