HEALTHY LAKEWOOD FOUNDATION WAS FORMED SEPTEMBER 7, 2018. NO GRANTS HAVE BEEN AWARDED AS THE ORGANIZATION IS IN FORMATION STAGE. THE VISION OF HEALTHY LAKEWOOD FOUNDATION IS "LAKEWOOD IS A PLACE WHERE ALL PEOPLE ACHIEVE THEIR FULLEST HEALTH POTENTIAL". THE HEALTHY LAKEWOOD FOUNDATION (HLF) DEMONSTRATED STRONG FINANCIAL MANAGEMENT IN 2024, WITH INVESTMENT ASSETS EXCEEDING 26 MILLION AS OF AUGUST 31. THE FOUNDATION RECEIVED ITS FOURTH BIENNIAL CONTRIBUTION OF THE REMAINING LAKEWOOD HOSPITAL ASSOCIATION ASSETS AND MAINTAINED A 5% SPENDING POLICY BASED ON A 13-QUARTER AVERAGE OF ASSETS. OVERSIGHT WAS PROVIDED THROUGH FIVE STANDING COMMITTEES: EXECUTIVE, FINANCE, GOVERNANCE, COMMUNITY ENGAGEMENT, AND LEARNING, EVALUATION, AND ASSESSMENT, WHICH MET AT LEAST QUARTERLY AND DEVELOPED TO ACHIEVE ANNUAL PRIORITIES. ANNUAL OFFICER ELECTIONS AND BOARD TERM RENEWALS WERE CONDUCTED AT THE MARCH ANNUAL MEETING, MAINTAINING GOVERNANCE CONTINUITY AND OVERSIGHT OF THE FOUNDATION'S EXPANDING COMMUNITY IMPACT. DUE TO ATTRITION, THE FOUNDATION ADDED ONE NEW BOARD MEMBER. IN 2024, HLF AWARDED GRANTS ACROSS THREE PROGRAMS TOTALING OVER 537,300: 212,500 IN COMMUNITY GRANTS SUPPORTING TWELVE ORGANIZATIONS' PROGRAMMATIC WORK; 104,800 IN NEIGHBORHOOD OPPORTUNITY GRANTS FUNDING TWENTY-ONE GRASSROOTS PROJECTS; AND 220,000 IN SYSTEMS-CHANGE GRANTS ADDRESSING SYSTEMIC ISSUES INCLUDING FOOD SECURITY, AFFORDABLE CHILDCARE, LEGAL REPRESENTATION, AND COMMUNITY JOURNALISM. THE FOUNDATION CONTINUED TO STRENGTHEN ITS COMMUNITY PRESENCE THROUGH ENHANCED COMMUNICATIONS STRATEGIES. IN SEPTEMBER, HLF HELD ITS SIXTH ANNUAL COMMUNITY MEETING, DRAWING 75 ATTENDEES FOR DISCUSSIONS ON PHILANTHROPY AND COMMUNITY BUILDING. HLF MAINTAINED ACTIVE COMMUNITY INVOLVEMENT THROUGH ITS EXECUTIVE DIRECTOR'S PARTICIPATION IN THE CITY OF LAKEWOOD'S COMMUNITY HEALTH ACTION TEAM (CHAT), SUPPORTING INITIATIVES TO BUILD A CULTURE OF HEALTH ACTION IN LAKEWOOD. A SIGNIFICANT ACHIEVEMENT WAS THE DEVELOPMENT OF THE LAKEWOOD COMMUNITY RESOURCE GUIDE, CREATED IN RESPONSE TO IDENTIFIED COMMUNITY NEEDS. THE FOUNDATION DEVELOPED AND IMPLEMENTED A GUIDING FRAMEWORK FOR ITS GRANTMAKING AND OPERATIONS, WHICH THE BOARD APPROVED IN NOVEMBER.
