Healthy Heart Communities (2HC) Program
The Healthy Hearts Communities Program (2HC) is a comprehensive healthcare initiative designed to help individuals and their families manage and prevent high blood pressure. The program utilizes a culturally responsive, community-based, and technology-driven approach to deliver healthcare support, education, and guidance. One of the key components of the 2HC program is Remote Patient Monitoring (RPM), which allows for the monitoring of vital signs, including blood pressure, using monitoring devices and services. Participants at risk of or living with diabetes may also receive blood glucose monitoring. This data is automatically communicated to the program team, ensuring proactive monitoring and support. The program operates through four pillars: Care Support at Home: A Community Care Team comprising healthcare professionals provides personalized clinical care support to participants. They teach participants how to self-monitor blood pressure at home and provide monitoring systems. The team leverages audiovisual resources and digital health technology to enhance health education and remote care support. Regular in-person or virtual visits, along with various communication channels, ensure ongoing support and monitoring. Lifestyle Behavioral Change Support: The program promotes a healthy lifestyle by encouraging regular physical activity and offering personalized exercise plans tailored to participants' needs. Nutrition counseling and education, along with access to heart-healthy diets, support weight management. Sleep improvement guidance, smoking cessation resources, and alcohol abuse support are also provided. Health Literacy and Well-being: Emotional well-being is emphasized through access to mental health counseling, social workers, and activities groups. Thematic health lessons cover topics such as self-care management, nutrition, and advocacy. Multiple communication channels, including videos, printed materials, and educational resources, facilitate effective education and coaching. Social Enhancements: Social support and networks play a vital role in managing hypertension. Participants are linked to peers, program alumni, and local health professionals to foster peer support and engagement in community activities. Regular support group activities, virtual communities, and information sessions promote a sense of belonging and active lifestyles. The expected outcomes of the program include care optimization, improved patient compliance, medication adherence, patient-physician relationships, increased physical endurance, healthier weight management, better emotional well-being, improved knowledge of hypertension, and the development of support networks. Through the Community Blood Pressure Screening and Health Education Project, Heart of a Giant will offer blood pressure screening and training on home blood pressure monitoring, distribute heart educational materials, and provide tips on healthy eating and physical activities. The Health Education classes will be provided to a group of individuals from Dorchester and Mattapan (of varying social and health backgrounds) for free. Individuals will be selected via sign-up through social media and online channels. Promotions will be done primarily through word-of-mouth, social media, local health centers such as the YMCA, and referrals within our existing medical and community networks. These Health Education classes will be delivered online following the “Empowered to Serve” toolkit.
