Lactation and Nutrition Consultation
Breast Friends will provide clinical lactation and nutrition consultation to providers and patients with young children zero to three who access primary care in the clinic based in Oakland CA. Breast Friends experts will: Consult with program patients nurse practitioners social workers and physicians on patient and family treatment plans. Facilitate monthly nutrition milk feeding and lactation education and support groups for Black identified families. Conduct outreach and in-reach to families and providers to support group patient enrolment in aforementioned education and support groups and conduct post-group follow-up with participants. Conduct pre-and post-group evaluation for continuous quality improvement. Provide education to the team as a whole during Breast Friends clinical service days. Advise the program study team on evaluation research related to lactation milk feeding and nutrition services. Complete clinical documentation and administrative duties including charting and billing provided that appropriate access to do so is provided. Adapt Breast Friends program and services tailored to the program clinic needs and priorities. Develop and provide written patient and provider education instructional and marketing materials clinical workflows outreach and plans and forms for Breast Friends enrollment. Breast Friends will provide services in the program clinic during regular clinic hours every Friday for 5 hours hours subject to change as appropriate based on patient and provider feedback. Breast Friends will participate in team meetings in-person 1 hour before the 5 hour clinic hours every Friday time subject to change as well as facilitated provider wellness and trauma-informed care workshops on the first and third Friday of every month. Breast Friends will also provide patients with at home support. Breast Friends will provide a total of eight hours of service every Friday.
