Short written narrative. Attach copy of IRS Exemption letter. Usually limit grant request to no more than $5,000.00

Sept 30, for Year Ending December 31
Any restrictions or limitations on awards, such as by geographical areas, charitable fields, or kinds of institutions

Grants generally do not exceed $5,000. Preferred location of grant recipients in Tucson, Arizona. Foundation encourages request from organizations assisting physically or developmentally handicapped.
