To educate enrich and assist senior citizens and their families as they make the transition from independent to assisted living arrangements. We worked with seniors with memory care and cognitive decline to navigate their changed circumstances and provided support to caregivers and families coping with the added responsibilities of caring for someone in their home. Provided education and support to families about dementia its stages behaviors and advised possible medications to ask doctors about to help control disruptive behaviors. Helped seniors apply for Medicaid or other state programs. Helped set appointments with Medicare and supplemental insurance carriers to review coverages. Called local home health care agencies to find qualified caregivers for families to interview as well as guide them in what to ask and what to look for in a quality healthcare worker.
