PACE IS A PROGRAM WHICH PROVIDES COMPREHENSIVE MEDICAL AND SUPPORTIVE SERVICES. THE INTERDISCIPLINARY TEAM AT PACE ASSESSES POTENTIAL PARTICIPANTS AND DEVELOPS A PERSON-CENTERED CARE PLAN - CUSTOMIZING SERVICES TO MEET INDIVIDUALS' NEEDS. FREQUENT CONTACTS WITH PARTICIPANTS AND REGULAR REVIEWS OF THE CARE PLAN ENSURE THAT PARTICIPANTS' NEEDS ARE BEING MET. PACE SEEKS TO FORM A PARTNERSHIP WITH THE PARTICIPANT AND CAREGIVERS TO PROMOTE INDEPENDENCE AND QUALITY OF LIFE.
