1.DOCUMENTATION OF TAX-EXEMPT STATUS PURSUANT TO A 501(C) (3), INCLUDING IRS LETTER OF DETERMINATION. 2.AN EMAIL ADDRESS FOR RECEIVING FUTURE COMMUNICATION. 3.YOUR ORGANIZATION'S MISSION STATEMENT AND PURPOSE. 4.YOUR ORGANIZATION'S BYLAWS AND GOVERNING DOCUMENTS. 5.A SUMMARY OF YOUR ORGANIZATION'S CHARITABLE PURPOSE. 6.A SUMMARY OF YOUR ORGANIZATION'S CRITERIA FOR DISBURSING FUNDS. 7.A RECORD DOCUMENTING PROPOSED EXPENDITURES RELATED TO THE REQUESTED FUNDING. 8.FINANCIAL STATEMENTS, INCLUDING BALANCE SHEET AND INCOME STATEMENT. 9. IF REQUESTING OVER 25,000 OR OVER IN FUNDING, AUDITED FINANCIAL STATEMENTS. 10. INCLUDE A COPY OF YOUR LATEST 990 TAX FILING.
