Choice Integrated HealthCare INC Choice Integrated HealthCare is a nonprofit behavioral health organization providing integrated mental health, substance use, and crisis services to individuals and families in Arizona, including underserved and high-trauma populations. Typically CALOCUS/LOCUS level 4. During the reporting year, the organization delivered comprehensive outpatient behavioral health services focused on accurate diagnosis, trauma-informed care, and continuity across levels of care. Services included psychiatric evaluation, medication management, individual and group counseling, case management, care coordination, and social determinants of health support. The organization operates under an integrated clinical model designed to improve diagnostic accuracy and reduce fragmented treatment. Comprehensive behavioral health assessments are conducted using extended evaluation protocols, incorporating mental health history, medical history, trauma history, social determinants of health, risk assessment, and standardized clinical tools. This model is designed to reduce misdiagnosis, improve treatment alignment, and decrease repeated entry into higher levels of care. The organization provided services to Medicaid, Medicare, commercially insured, and uninsured individuals, with a significant proportion of services delivered to underserved and high-risk populations, including Native communities and individuals experiencing significant trauma exposure. Core program accomplishments include: Delivery of integrated outpatient behavioral health services to individuals experiencing serious mental illness, substance use disorders, trauma-related disorders, and co-occurring conditions. Implementation of a structured non-confrontational, trauma-informed care model designed to preserve dignity and reduce escalation in high-acuity environments. Development and implementation of a comprehensive assessment system designed to meet and exceed evolving state standards for behavioral health evaluation. Coordination of care across providers and levels of service to support continuity and reduce gaps in treatment. Establishment of partnerships with community stakeholders to expand access to behavioral health services in underserved regions.
