Youth and Family Services Program
Youth and Family Services Program:Youth and Family Services provides a continuum of support to youth with disabilities, their families, and unaccompanied youth, assisting them to identify goals and create a plan of how to achieve them. Support activities across programs include assessing needs and strengths, identifying desired outcomes, making referrals, attending related meetings with clients, facilitating a team-based process and plan of care, engaging collaboratively with other service providers, assisting clients with applications for services, stabilizing education plans, and engaging natural supports.
