JOSEPH HEALTH (PROVIDENCE). BY COMING TOGETHER, PROVIDENCE SEEKS TO BETTER SERVE ITS COMMUNITIES THROUGH GREATER PATIENT AFFORDABILITY, OUTSTANDING CLINICAL CARE, IMPROVEMENTS TO THE PATIENT EXPERIENCE AND INTRODUCTION OF NEW SERVICES WHERE THEY ARE NEEDED MOST. TOGETHER, OUR CAREGIVERS SERVE IN 51 HOSPITALS, 1,085 CLINICS ACROSS ALASKA, CALIFORNIA, MONTANA, NEW MEXICO, OREGON, TEXAS AND WASHINGTON. THE FOUNDERS OF BOTH ORGANIZATIONS WERE COURAGEOUS WOMEN AHEAD OF THEIR TIME. THE SISTERS OF PROVIDENCE AND THE SISTERS OF ST. JOSEPH OF ORANGE BROUGHT HEALTH CARE AND OTHER SOCIAL SERVICES TO THE AMERICAN WEST WHEN IT WAS STILL A RUGGED, UNTAMED FRONTIER. NOW, AS WE FACE A DIFFERENT LANDSCAPE A CHANGING HEALTH CARE ENVIRONMENT WE DRAW UPON THEIR PIONEERING AND COMPASSIONATE SPIRIT TO PLAN FOR THE NEXT CENTURY OF HEALTH CARE.PROVIDENCE HEALTH & SERVICESIN 1856, MOTHER JOSEPH AND FOUR SISTERS OF PROVIDENCE ESTABLISHED HOSPITALS, SCHOOLS AND ORPHANAGES ACROSS THE NORTHWEST. OVER THE YEARS, OTHER CATHOLIC SISTERS TRANSFERRED SPONSORSHIP OF THEIR MINISTRIES TO PROVIDENCE, INCLUDING THE LITTLE COMPANY OF MARY, DOMINICANS AND CHARITY OF LEAVENWORTH. RECENTLY, SWEDISH HEALTH SERVICES, KADLEC REGIONAL MEDICAL CENTER AND PACIFIC MEDICAL CENTERS HAVE JOINED PROVIDENCE AS SECULAR PARTNERS WITH A COMMON COMMITMENT TO SERVING ALL MEMBERS OF THE COMMUNITY. ST. JOSEPH HEALTH SYSTEMIN 1912, A SMALL GROUP OF SISTERS OF ST. JOSEPH LANDED ON THE RUGGED SHORES OF EUREKA, CALIFORNIA TO PROVIDE EDUCATION AND HEALTH CARE. THEY LATER ESTABLISHED ROOTS IN ORANGE, CALIFORNIA, AND EXPANDED TO SERVE SOUTHERN CALIFORNIA, NORTHERN CALIFORNIA AND TEXAS. THE HEALTH SYSTEM ESTABLISHED MANY KEY PARTNERSHIPS, INCLUDING A MERGER BETWEEN LUBBOCK METHODIST HOSPITAL SYSTEM AND ST. MARY HOSPITAL TO FORM COVENANT HEALTH IN LUBBOCK, TEXAS. RECENTLY, AN AFFILIATION WAS ESTABLISHED WITH HOAG HEALTH TO INCREASE ACCESS TO SERVICES IN ORANGE COUNTY, CALIFORNIA.PROVIDENCE HEALTH PLANS (PHP) OFFERS VARIOUS COMMERCIAL GROUP AND INDIVIDUAL AND FAMILY PLANS, THE MAJORITY OF WHICH ARE ACA INDIVIDUAL AND SMALL GROUP PLANS, THROUGHOUT OREGON AND WASHINGTON. GROUP COVERAGE IS OFFERED TO EMPLOYERS WHO PROVIDE ACCESS TO HEALTH COVERAGE TO THEIR EMPLOYEES, WHILE INDIVIDUAL AND FAMILY PLANS ARE OFFERED BOTH ON AND OFF OF THE PUBLIC EXCHANGES. TO DELIVER TRUE HEALTH TO OUR MEMBERS, PHP FOCUSES ON INNOVATION, PERSONALIZED MEDICINE, COLLABORATION AMONG DIVERSE PARTNERS, AND STABILITY. PHP ALSO PARTICIPATES IN HEALTH MEASUREMENT PROGRAMS, INCLUDING HEALTHCARE EFFECTIVENESS DATA AND INFORMATION SET (HEDIS), WHICH ALLOW US TO BENCHMARK OUR CARE AND SERVICE, AND IMPLEMENT A TARGETED APPROACH TO IMPROVING MEMBER HEALTH AND SERVICE. AS A RESULT OF COVID PROVISIONS PUT INTO PLACE BY QUALITY REGULATORS, UPDATED RATINGS WERE NOT ISSUED, AND PRIOR YEAR RATINGS WERE CARRIED FORWARD. AS A RESULT, PHP'S OREGON COMMERCIAL LINE OF BUSINESS RECEIVED A 4.0 RATING FROM THE NATIONAL COMMITTEE FOR QUALITY ASSURANCE (NCQA) FOR THE 2020-2021 RATINGS. PROVIDENCE CONTINUES TO BE RECOGNIZED AS THE HIGHEST RATED COMMERCIAL PPO PLAN IN OREGON. FOR PLAN YEAR 2020, PHP'S OREGON EXCHANGE LINE OF BUSINESS RECEIVED A 4.0 RATING FOR THE QUALITY RATING SYSTEM (QRS), AND NO HEALTH PLAN IN OREGON RECEIVED A HIGHER MARKETPLACE RATING THAN DURING 2020, PHP SERVED APPROXIMATELY 179,000 MEMBERS.PROVIDENCE OFFERS PROVIDENCE ADMINISTRATIVE SERVICES ONLY (ASO) FOR LARGE GROUPS CAPABLE OF SELF-FUNDING THE RISK EXPERIENCE OF THE GROUP, BUT ALSO WANT THE DEDICATED AND COMPASSIONATE SERVICE THAT PROVIDENCE DELIVERS FOR CLAIMS ADMINISTRATION, MEMBER SERVICE, AND SKILLED MEDICAL MANAGEMENT. ASO GROUPS ARE OFFERED OPTIONS FOR A WIDE ARRAY OF PLAN BENEFITS, INCLUDING PREVENTATIVE CHOICES AIMED AT IMPROVING EMPLOYEE HEALTH AND WELLNESS, ALTERNATIVE CARE, GROUP HEALTH REPORTING AND METRICS, ACCESS TO PROVIDENCE PROVIDER NETWORKS AND PROPRIETARY HEALTH INNOVATIONS, AND CONSUMER CHOICE PLANS WHICH OFFER QUALIFIED HEALTH SAVINGS ACCOUNT (HSA) OPTIONS. THIS PROGRAM SERVED APPROXIMATELY 294,000 MEMBERS IN 2020.GRANTS AND ALLOCATIONS TO 501(C)(4) ORGANIZATIONS: SEE SCHEDULE IAT PROVIDENCE, RESPONDING TO COMMUNITY NEEDS, ESPECIALLY IN TIMES OF CRISIS, HAS ALWAYS BEEN AT THE HEART OF THE HEALING CARE PROVIDED BY OUR FAMILY OF ORGANIZATIONS. TOGETHER, PROVIDENCE TEAMS RESPONDED TO THE COVID-19 PANDEMIC WITH ALL HANDS ON DECK. THE RESOURCES NEEDED WERE UNPRECEDENTED, AND THE IMPACT ON ALL HOSPITALS AND HEALTH SYSTEMS HAS BEEN SIGNIFICANT.PROVIDENCE HEALTH PLAN'S PRIORITY REMAINS THE HEALTH OF OUR MEMBERS AND OUR COMMUNITIES. WE AIM TO PROVIDE HOPE AND COMFORT WITH RESOURCES, NEWS AND GUIDANCE ON COVID-19. WE HAVE ALSO TAKEN THE NECESSARY MEASURES TO SUPPORT OUR HEALTH PLAN MEMBERS, PROVIDERS, EMPLOYERS AND BROKER PARTNERS WHO ARE IMPACTED DURING THIS DIFFICULT TIME. IN ADDITION, AS PART OF AN INTEGRATED DELIVERY SYSTEM, OUR TEAM OF CUTTING-EDGE MEDICAL EXPERTS DEVELOPED A HUB OF COVID-19 RELATED RESOURCES, INCLUDING A CORONAVIRUS ASSESSMENT TOOL, WELLNESS RESOURCES FOR MENTAL AND PHYSICAL HEALTH, INFORMATION AND UPDATES DIRECTLY FROM PROVIDENCE EXPERTS AND RESEARCHERS, AND MORE AT HTTPS://CORONAVIRUS.PROVIDENCE.ORG/.PROVIDENCE HEALTH PLAN HAS ALSO TAKEN CONCRETE ACTIONS IN ITS COVERAGE AND OPERATIONS TO SUPPORT ITS MEMBERS, PROVIDERS, BUSINESS PARTNERS AND LOCAL COMMUNITIES:- PROVIDENCE HAS TAKEN THE INITIATIVE TO HELP ENSURE HEALTHCARE WORKERS HAVE THE NECESSARY PERSONAL PROTECTIVE EQUIPMENT (PPE), AND TO PROVIDE VULNERABLE POPULATIONS WITH SOAP AND DISINFECTANT TO HELP PREVENT THE SPREAD OF COVID-19. - COST SHARING FOR COMMERCIAL INSURED MEMBERS FOR TREATMENT OF COVID-19 WAS WAIVED FOR IN-NETWORK INPATIENT, OUTPATIENT, FACILITY AND PROFESSIONAL VISITS. - PROVIDENCE EXPANDED ITS HEALTH PLAN PREMIUM PAYMENT GRACE PERIOD FOR COMMERCIAL GROUPS SO MEMBERS CAN CONTINUE TO RECEIVE CARE AND MEDICATIONS DURING THIS TIME OF UNCERTAINTY. WE ALLOWED COMMERCIAL GROUPS TO MAINTAIN COVERAGE FOR THEIR FURLOUGHED EMPLOYEES AT THE GROUP PREMIUM RATE.- PHP WAIVED ALL COST SHARING FOR TESTING SERVICES RELATED TO COVID-19, SUCH AS COPAYS, COINSURANCE, AND DEDUCTIBLES.- OUR TELEHEALTH PROVIDER POLICIES WERE SIGNIFICANTLY EXPANDED TO REDUCE BARRIERS TO CARE AND TO ENCOURAGE THE USE OF TELEHEALTH SERVICES BY OUR MEMBERS AND PROVIDER PARTNERS. WE HAVE ALSO ADDED VIRTUAL CAPACITY SO THAT MORE OF OUR PROVIDERS CAN BE SEEN QUICKLY.- PHP DEVELOPED AND IMPLEMENTED A CORONAVIRUS ASSESSMENT TOOL TO HELP OUR COMMUNITIES ASSESS THEIR SYMPTOMS IN REAL TIME.- OUR LABS IN OREGON BEGAN TESTING FOR CORONAVIRUS EARLY ON, BECOMING THE FIRST HOSPITAL SYSTEM IN THE STATE TO OFFER IN-HOUSE LAB ANALYSIS AND INCREASING OUR COMMUNITY'S TESTING CAPACITY.- AT THE END OF THE YEAR, THE PLAN ISSUED PREMIUM CREDITS TO MEMBERS WITH AN INDIVIDUAL PLAN, TOTALING MORE THAN $1.5 MILLION.MANY INDIVIDUALS AND BUSINESSES IN OREGON WERE DEVASTATED BY WILDFIRES IN 2020 AS THEY ALSO COPED WITH THE CHALLENGES POSED BY COVID-19. AS A RESULT OF THE STATE OF EMERGENCY DECLARED IN OREGON, PROVIDENCE TOOK CONCRETE ACTIONS IN ITS COVERAGE AND OPERATIONS TO SUPPORT ITS MEMBERS THAT LIVE IN AFFECTED AREAS. THESE INCLUDE:- MEMBERS UNABLE TO ACCESS ANY IN-NETWORK PROVIDERS DUE TO THE FIRES WERE ALLOWED ACCESS TO OUT-OF-NETWORK PROVIDERS AT IN-NETWORK COST SHARE.- MEMBERS WERE ALLOWED REFILLS OF DIABETIC SUPPLIES AND MAINTENANCE MEDICATIONS EARLIER THAN SCHEDULED AND THE EXTENSION OF MEDICATION PRIOR AUTHORIZATION APPROVALS.- DURABLE MEDICAL EQUIPMENT: MEMBERS MAY REPLACE ITEMS (FOR EXAMPLE, EYEGLASSES, HEARING AIDS, AND ORTHOTICS.) THAT WERE LOST OR DAMAGED BECAUSE OF THE WILDFIRES. - AN EXTENSION ON THE PREMIUM PAYMENT GRACE PERIOD THROUGH THE END OF THE YEAR (DEC 31) FOR COMMERCIAL GROUPS AND INDIVIDUAL MEMBERS. MEMBERS WERE OFFERED INTERVENTIONS INCLUDING: - ENROLLMENT INTO BOTH CARE MANAGEMENT AND HEALTH COACHING TO WORK WITH A CERTIFIED DIABETES EDUCATOR AND REGISTERED DIETICIAN.- TARGETED EDUCATION SPECIFIC TO DIABETES AND/OR HYPERTENSION.- DIABETES GOAL SETTING.- MEDICATION INTERVENTIONS INCLUDING IDENTIFICATION OF BARRIERS AND GOAL SETTING.- COORDINATION OF MEMBER COMMUNITY OR RESOURCE NEEDS AS IDENTIFIED.- PROVIDER INTERVENTIONS TO SUPPORT IDENTIFYING BARRIERS AND CONNECTING MEMBERS TO PRIMARY CARE, SPECIALTY OR BEHAVIORAL HEALTH PROVIDERS.- ADDITIONAL AREAS OF FOCUS IN COHORT 2:- STATIN THERAPY: IDENTIFYING AND EDUCATING MEMBERS ON IMPORTANCE OF STATIN THERAPY.- FOOD DISPARITY INTERVENTION REFERRAL TO MEMBERS WITH FOOD INSECURITIES WHO LIVED IN GEOGRAPHIC SCOPE.- RETINAL EYE SCREENING: ENSURING MEMBERS COMPLETED OR HAVE SCHEDULED APPOINTMENTS FOR RETINAL EYE SCREENS.FOOD DISPARITY PILOTPROVIDENCE CARE MANAGEMENT FOOD DISPARITY PILOT OFFERED A TIERED APPROACH TO PROVIDING MEALS. MORE THAN 90 MEMBERS RECEIVED SUPPORT IN ONE OR MORE OF THE FOLLOWING WAYS:- INDEPENDENT MEAL PREP/GIFT CARD TIER.- MEMBER MAILED A $50/WEEK GIFT CARD TO USE ON FOOD (GIFT CARD RESTRICTIONS: TOBACCO, ALCOHOL, FIREARMS, LOTTERY).- PARTIAL DEPENDENT MEAL PREP/READY TO PREPARE MEALS TIER.- MEMBER RECEIVED READY TO PREPARE FOOD BOXES, DELIVERED BY ALBERTSONS/SAFEWAY CONTAINING 6 MEALS/WEEK (BOXES WILL CONTAIN 3 MEALS/2 SERVINGS = 6 MEALS AND CHOICE OF FRUIT).- UNABLE TO PREPARE MEALS/READY TO CONSUME MEALS.- MEMBER RECEIVED 6 READY TO EAT MEALS/WEEK AND CHOICE OF FRUIT, DELIVERED BY ALBERTSONS/SAFEWAY (CONTAINS 3 FRESH MEALS AND 3 FROZEN MEALS).82% OF MEMBERS WHO COMPLETED THE PROGRAM HAD AN IMPROVEMENT IN THEIR FOOD DISPARITY SCORE AND 90% OF MEMBERS SAID THAT THE INTERVENTION GAVE THEM CONFIDENCE TO PREPARE MEALS AND UTILIZE COMMUNITY RESOURCES.OUR TELEHEALTH PROVIDER POLICIES WERE SIGNIFICANTLY EXPANDED TO REDUCE BARRIERS TO CARE AND TO ENCOURAGE THE USE OF TELEHEALTH SERVICES BY OUR MEMBERS AND PROVIDER PARTNERS. WE ALSO ADDED VIRTUAL CAPACITY SO THAT MORE OF OUR PROVIDERS CAN BE SEEN QUICKLY.THE BEHAVIORAL HEALTH CONCIERGE PROGRAM ALLOWED MEMBERS SUFFERING FROM BEHAVIORAL HEALTH ISSUES TO BE SEEN MORE QUICKLY MORE THAN 70% OF PEOPLE UTILIZING THE SERVICE WERE SEEN WITHING 48HRS OF THEIR REQUEST.
