Application is available online at the Foundation's website. It is submitted through an online portal. Information required includes contact information, child's information including diagnosis and date of diagnosis, primary care physician, oncologist, applicant's favorite activities,clothing size, etc.

Awards are ginve for three month period and are to be used as supplemental income by the family to assist with necessary care, bills and medical expenses. In addition to the application, each family must complete a financial form that lists the applicants income, fixed expenses debt, etc.
