SIMPLE Diabetes Management ProgramIn this program, community health workers educate individuals with diabetes to understand and overcome common community healthcare barriers. The program provides monthly in-person classes and remote classes and is designed with monthly videos that can be sent to patients. In this curriculum, we share ideas of questions that community health workers can ask patients weekly related to the month's video.
