There is a written application form. Information to be included is a brief summary of the organization's history, the organization's mission and goals, a brief description of the organization's current programs and activities, any relationship with other organizations working similar missions, the number of board members, full-time paid staff, part-time paid staff and volunteers, a description of the overall goals of the organization for the funding period, objectives or ways in which the organization will meet the goals, activities and who will carry out these activities, the time frame in which this will take place, how the proposed activities will benefit the community in which they will occur, long term funding strategies of the organization, and a description of how the proposal meets the Trust's geographical requirements (benefiting the residents of Carroll and Sac Counties in the State of Iowa).
