SERVICES ARE PROVIDED AT THREE FREESTANDING CLINICS: FREMONT CLINIC IN NORTH MINNEAPOLIS, CENTRAL AVENUE CLINIC AND SHERIDAN WOMEN & CHILDREN'S CLINIC IN NORTHEAST MINNEAPOLIS. ON-SITE SERVICES INCLUDE PRIMARY CARE, PREVENTION, OUTREACH, OB/GYN, CHRONIC DISEASE, INTERPRETING, AND ASSISTANCE WITH HEALTH CARE PLAN ENROLLMENT. WE PROVIDE REFERRALS AND CARE COORDINATION FOR DENTAL, MENTAL HEALTH AND SUBSTANCE ABUSE, HOSPITAL, AND SPECIALTY SERVICES THROUGH PARTNERSHIPS WITH OTHER COMMUNITY ORGANIZATIONS.
