POTENTIAL GRANTEES ARE REQUIRED TO CONTACT THE EXECUTIVE DIRECTOR AND GO THROUGH AN INITIAL SCREENING PROCESS VIA TELEPHONE TO DETERMINE IF THE ORGANIZATION IS WITHIN THE PARAMETERS OF THE FOUNDATION'S MISSION AND APPROPRIATE FOR FUNDING. THE APPLICATION IS AVAILABLE ON THE FOUNDATION'S WEBSITE. GRANT SUBMISSIONS ARE SUBMITTED VIA MAIL TO THE FOUNDATION'S PO BOX AND ELECTRONICALLY, IN ADVANCE OF THE DEADLINE. THE EXECUTIVE DIRECTOR THEN REVIEWS THE APPLICATIONS FOR COMPLETENESS, WRITES A SUMMARY AND DISTRIBUTES A LIST OF ALL SUBMISSIONS AND THE AMOUNT OF ALL GRANT REQUESTS TO THE BOARD OF DIRECTORS. SITE VISITS ARE CONDUCTED BY BOARD MEMBERS. THE FOLLOWING ITEMS ARE REQUIRED AS PART OF THE APPLICATION PROCESS: A COMPLETED STANDARD APPLICATION FORM, IRS DETERMINATION LETTER, 1ST PAGE OF FORM 990, MOST RECENTLY AUDITED FINANCIAL STATEMENTS, PROJECT BUDGET, LIST OF OTHER FUNDING SOURCES, ORGANIZATION BUDGET AND PROJECT TIMELINE.
