Family Tree Clinic Services
Clinic:Family Tree Clinic is a nonprofit community clinic established to provide health services to populations most impacted by health inequities, including low-income persons, young people, LGBTQ communities and people of color. To increase accessibility, Family Tree offers services on a sliding-fee scale; no one is turned away for an inability to pay. Family Tree is designated an Essential Community Provider by the state and is part of the Twin Cities health care safety net.Since 1971, Family Tree Clinic has offered innovative clinical and education services focused on sexual health and that center around respect, cultural competence, and individuality. For over fifty-one years, Family Tree has confronted barriers to health care head-on, and today is a regional and national leader in inclusive sexual health care and education.The mission of Family Tree is to cultivate a healthy community through comprehensive sexual health care and education. Our vision is to eliminate health disparities through innovative, personalized sexual health care and education for diverse needs. Family Tree advances its mission and vision by providing: medical reproductive and sexual health care services; limited primary care; culturally responsive services to the Lesbian, Gay, Bisexual, Transgender, and Queer (LGBTQ) community; community health education programming; the MN Family Planning + STD Hotline; interdisciplinary training to health care professionals on inclusive practices; authentic community engagement that informs services; and collaborative partnerships with other service organizations.Family Tree Clinic is guided by its strategic plan, which was approved by its Board of Directors and builds on Family Tree's long-standing commitment to improving the health and wellbeing of Twin Cities residents, particularly those who are underserved, experience health disparities, and face barriers to accessing care. Key goals for the current plan are: 1) Provide national leadership focused on education, advocacy and policy on sexual health care; 2) Reduce health disparities through accessible, sex-positive health care and education; 3) Expand services and programs to better connect the community with coordinated, quality care and education grounded in sexual and reproductive health; and 4) Increase Family Tree Clinic's organizational capacity to successfully carry out its work.Family Tree Clinic's Board of Directors will undergo a strategic planning process in 2023 to identify strategic priorities and develop new long-term goals for the coming years. The strategic plan will further develop Family Tree's new sexual health and education center in Minneapolis as a beacon of health and wellbeing in the Twin Cities and the region. Family Tree Clinic delivers on its mission through four core programs: 1) culturally responsive medical services centering people most impacted by health inequities; 2) comprehensive sexuality and healthy relationships education in schools, juvenile detention centers, and community locations; 3) Community engagement that creates authentic pathways for the community to inform and shape the services we provide, with a focus on intersectional Black communities; and 4) Interdisciplinary training for health care professionals and organizations, with a focus on LGBTQ-competency in health care delivery. Family Tree believes that patient-centered medical care, upstream education, authentic community engagement, and training of the health care workforce will result in health equity for people most on the margins. Family Tree serves over 20,000 people in a typical year through all programs and community engagement, and is an established statewide and regional leader in the field of LGBTQ Health Equity and Inclusive Care Models. We received a perfect score on HRC's LGBT Healthcare Equality Index in 2022, a distinction afforded to only five organizations statewide. Family Tree offers one of the only sliding-fee scale, informed consent Hormone Care Programs in the state. Trans folks choose our program for many reasons - we often hear that this is the only clinic where they feel safe, seen, respected, affirmed and like they are partners in their care. Family Tree provides preventive primary medical care services, with a focus on reproductive and sexual health care. Medical services include: pap smears and annual preventive exams; colposcopy and breast/chest exams; mammogram referrals; menopause care; STI testing and treatment; HIV care including screening, testing, and PrEP for HIV prevention; treatment for UTIs and other infections; preconception counseling; pregnancy testing and all-options counseling; acute primary care including for upper respiratory infections, skin conditions, and immunizations; and an informed consent trans hormone care program.Patients may pay for these programs using private insurance, government programs or grants, or according to Family Tree's sliding-fee scale. To ensure equitable access, we do not turn anyone away for an inability to pay. Family Tree provides services primarily to individuals in the 7-county metro area, many patients travel from across the state and region to receive the high quality care available at Family Tree Clinic. Family Tree Clinic's target populations are low-income individuals, individuals who are uninsured or underinsured, young people, individuals facing barriers to care due to stigma, confidentiality concerns, and lack of cultural competence, people of color, and LGBTQ community members. In 2022, Family Tree Clinic provided 5,768 visits to 2,357 unduplicated patients (a 10% increase in patients from the previous year). 76% of patients identified as LGBTQ and 32% as BIPOC. 47% of patients were under the age of 25. 42% of patients were living below the federal poverty level (FPL) and another 24% were living below 200% of the FPL. Additionally, 17% of patients were uninsured and another 29% used Medicaid or MNCare as their primary insurance. To increase access, Family Tree offers MNsure navigation and resources coordination. Family Tree's MNsure navigators work with uninsured clients to enroll them in health insurance programs, including Minnesota Health Care Programs, private insurance options, and a Medicaid waiver program called the Minnesota Family Planning Program (MFPP). In 2022, 194 patients enrolled in health insurance with assistance from Family Tree, and an additional 70 received access to MNsure Navigation services. In 2022, Family Tree Clinic created its new Midwest Trans Health Education Network - a remote cohort of providers in rural areas of the upper Midwest who will receive training and mentorship from Family Tree on trans and gender expansive health care competency, so that they may provide this essential health care in their home communities - reducing geographic barriers to care for patients. In addition to growing a network of rural providers trained in trans and gender expansive health care, Family Tree Clinic is expanding its capacity as a destination clinic to meet increased demand for services. Health Education Program:Family Tree's Community Health Education program also saw an increase in participation in 2022. Family Tree's Health Education program fills an urgent public health need by providing comprehensive sexuality and healthy relationships education to youth, adults and families in schools, juvenile detention centers, and community locations. Our comprehensive health education aims to positively influence sexual health by helping participants feel comfortable with their bodies, confidence in their knowledge, and powerful in their decision making abilities. Family Tree uses the FLASH curriculum (developed in part by Family Tree) for many lessons, which is nationally recognized and proven effective in increasing participants' confidence to abstain from sex or make safer sex choices. We also engage families through Parents are Sex Educators (PASE) lessons and Early Childhood Family Education (ECFE) classes. In 2022, Family Tree served over 7,400 unduplicated individuals through comprehensive health education.Family Tree's Community Engagement program reaches people at community locations, outside of medical settings, cultivating authentic relationships within communities to build trust before connecting people to the full range of quality, affirming health services available through the clinic. Community Engagement staff hold listening sessions within communities to discuss health access, provide HIV/STI testing resources, group Parents are Sex Educators lessons, and educational and medical resources.Since 1979, Family Tree has operated the Department of Health-funded MN Family Planning + STD Hotline (1-800-78-FACTS, www.sexualhealthmn.org), a statewide service providing free and confidential information about STIs, birth control, and family planning. Through the hotline, we also maintain a family planning provider database and provide referrals to low-cost clinics in all 87 counties in Minnesota.Family Tree launched several new initiatives in 2022, including a drop-in program for LGBTQ youth in partnership with Avenues for Youth, a free LARC initiative, and the Midwest Trans Health Education Network. Using community feedback from listening sessions, our Community Engagement Department partnered with Macalester College on a research project aimed at making local health care systems more inclusive for intersectional Black communities.In 2023, Family Tree Clinic will continue providing quality, affordable sexual health and education services to communities most impacted by health inequities. In addition to core program, specific activities include:1. Provide reproductive and sexual health medical services reaching 3,000 unduplicated patients. Services are offered on a sliding-fee scale and include birth control, STI testing and treatment, limited primary care, hormone care, annual exams, etc.2. Provide 6,000 tests/screens for STIs through the clinic.3. Offer a full range of affordable, low-to no-cost birth control options to all, including long acting reversible contraception (LARC) methods.4. Provide medically accurate and age appropriate sexuality and healthy relationships education for 7,000 unduplicated youth and families in schools, juvenile detention centers, and at community locations.5. Provide 25 group sessions to youth in juvenile detention centers focused on family planning, STI/HIV prevention, and healthy relationships with Family Tree's Community Health Educators.6. Utilize Family Tree Clinic's trained MNsure Navigators on staff to assist uninsured patients with access to, and enrollment in, health coverage. 7. Enroll 150 individuals in the Minnesota Family Planning Program.8. Enhance patient access to HIV prevention resources including testing and PrEP, a daily pill that prevents transmission of HIV.9. Provide health resources and information to 2,500 community members through exhibits and outreach at community events.10. 100% of patients needing care beyond that available at Family Tree will be referred to other providers. Examples include management of hypertension and dental care.11. Cultivate clinical services and environments that better meet the needs for culturally competent sexual and reproductive health care for LGBTQ and BIPOC communities.12.
