Dementia Support Programs
Dementia Support Programs: Agrace offers several programs that teach and guide people who are living with dementia and their families. These programs include Dementia Care Consultations, our "Learning to Live with Dementia" education series, and daytime care for seniors with memory loss/dementia. Community support is vital to sustain these programs and keep fees low, so more families can learn how to navigate dementia with compassion and confidence. Clinical Advancement: Agrace supports the professional growth and development of our staff through several types of advancement opportunities. These include earning credentials and certifications for their roles and completing specialized training programs that can help them provide the best possible care to our patients and clients. With the support of generous community donors, we can also offer scholarships, mentorship, and a variety of unique learning experiences that enrich our staff's professional abilities. Patient Wish and Essential Needs Fund: This fund helps Agrace bring comfort, joy, and dignity to hospice patients by fulfilling meaningful wishes. Their wish may be to achieve a lifelong dream, a final desire, or something a patient could not afford but will improve their quality of life. Since 2010, Agrace has granted more than 225 wishes for our hospice patients. The fund also supports essential needs like gas cards, groceries, and communication devices for families. Additionally, it covers the costs for the Agrace staff to prepare and deliver holiday meals for our in-home patients and their families. CARE FOR ALL (CHARITY CARE): EACH YEAR, DOZENS OF OUR NEIGHBORS WHO CANNOT AFFORD HOSPICE, PALLIATIVE CARE, Age at Home, or the Adult DAy Center RECEIVE SERVICES AT NO COST THROUGH AGRACE'S "CARE FOR ALL" PROGRAM, funded by the Care for All endowment. Through the Care for All Program, WE SERVE SOME OF OUR MOST VULNERABLE COMMUNITY MEMBERS-CHILDREN AND YOUNG ADULTS WITH NO INSURANCE, SENIORS WITHOUT CAREGIVERS, INDIVIDUALS LACKING SAFE HOUSING, AND THOSE WITH SPECIAL NEEDS. While Medicare, Medicaid, and private health insurance cover the cost of basic hospice services for most patients, an increasing number of people in southern Wisconsin do not have enough insurance coverage for hospice when they need it. COMMUNITY GRIEF SERVICES: EVERY YEAR, THOUSANDS OF PEOPLE, INCLUDING CHILDREN, GET SUPPORT FROM AGRACE'S SPIRITUAL & GRIEF SERVICES TO HELP THEM COPE WITH THE DEATH OF A LOVED ONE. ALTHOUGH GRIEF SUPPORT IS REQUIRED BY STATE AND FEDERAL REGULATIONS, IT IS NOT COVERED BY MEDICARE, MEDICAID, OR PRIVATE INSURANCE. COMMUNITY DONATIONS ENABLE US TO OFFER SPECIALIZED GRIEF SUPPORT THAT IS OPEN TO EVERYONE. THE FOUNDATION ALSO SUPPORTS Agrace's free-standing GRIEF SUPPORT CENTER which HAS ALLOWED US TO EXPAND OUR PROGRAM AND SERVE MANY MORE PEOPLE. In 2024, Agrace's Community Grief Program supported 170 students in 46 schools.
