Senior Support and Resource Services
Service Accomplishments Maintain order and verify all records for the Focus Hope Food Program to ensure that seniors financially qualify to receive food supplements, provide resources for Meals on Wheels program. Provide resources for Medicare and Medicaid enrollment and changes. Partnership with the Plymouth Township Police department and Homeland Security to provide Scams and Fraud Seminars quarterly. Partner with the Plymouth Township Fire Department to provide Senior Lift Assists referrals, Sugar and Blood Pressure screening. Assist the Plymouth United Way with charity collections for the Homeless. Arranged for low income seniors to participate in the "Project Heat Program" (cleaning and maintaining furnaces). Held informative presentations regarding Diabetes Depression, Fall Prevention, Pain Management, Hearing testing monthly, Dementia, Home Care & Housing, Eye Screenings, Blood Pressure and Sugar Checks, monthly, Wellness and Managing Meds, Estate Planning, Posture Screening, Exercise & Nutrition. Provided resources for Adult Day Care and Elder Abuse. Provided resources for transportation, utilities, emergency medical trips, chore program, ramps, cell phones, home care and food supplements. Participants attended Social, Entertaining and instructional classes. We offer on a weekly basis Cardio Strength and Stretch, Toning with Tunes and YoChi classes. Editing, publishing and distributing 12 issues of the Sentinel Newsletter, to approximately 472 members and 24 senior agencies. The coordinating of seniors with local agencies to ensure that seniors receive assistance, current resources and assistance.
