FRANCOIS COUNTY BOARD FOR THE DEVELOPMENTALLY DISABLED, IS AN EIGHT BED MEDICAID WAIVER FACILITY THAT PROVIDES RESIDENTIAL HABILITATION SERVICES TO MEDICAID ELIGIBLE INDIVIDUALS WITH A DEVELOPMENTAL DISABILITY. THE ST. FRANCOUIS COUNTY BOARD FOR THE DEVELOPMENTALLY DISABLED PROVIDES SERVICES TO INDIVIDUALS WHO LIVE AT HOME OR IN RESIDENTIAL SETTINGS. THESE SUPPORTS CENTER AROUND THE INDIVIDUAL AND ARE DESIGNED TO FACILITATE A PERSON'S CHOICE TO WORK, LEARN AND ACTIVELY PARTICIPATE WITH NON-DISABLED PEERS IN THE COMMUNITY. THESE SUPPORTS MIGHT INCLUDE TRAINING, PROTECTIVE OVERSIGHT, PHYSICAL ASSISTANCE, ENVIRONMENTAL ADAPTIONS, ACCOMPANIMENT, COACHING, AND MINOR PROBLEM SOLVING TO ACHIEVE INCREASED INDEPENDENCE, PRODUCTIVITY, AND INCLUSION WITHIN THE COMMUNITY. ST. FRANCOIS COUNTY BOARD FOR THE DEVELOPMENTALLY DISABLED TARGETED CASE MANAGEMENT SERVICES PROVIDES SERVICE COORDINATION TO DEVELOPMENTALLY DISABLED CONSUMERS THROUGHOUT ST. FRANCOIS COUNTY AND CURRENTLY SERVES APPROXIMATELY 200 INDIVIDUALS. SERVICE COORDINATORS ARE RESPONSIBLE FOR ENSURING THAT INDIVIDUALS RECEIVING SUPPORT HAVE ACCESS TO AND RECEIVE SERVICES THAT MEET THEIR INDIVIDUAL NEEDS. THESE ARE ACCOMPLISHED BY FORMULATING A PERSONAL PLAN, INFORMING THE CONSUMERS OF AVAILABLE SERVICES AND THEN LINKING THE CONSUMER TO THOSE SERVICES FROM AGENCIES IN THE COMMUNITY.
