Financial assistance from The Jacks Family Foundation may be obtained by contacting the Foundation or any individual board member by mail or e-mail. The form of the application may be in a written or e-mail reply that includes basic contact information from the applicant, along with a description of how the funds will be used and how the funds will enhance the applicant's ability to promote Christian ministry. The Foundation may request additional information, including but not limited to, applicant's tax-exempt status verification, organizing documents, program/project budget, organization's financial statements, and other relevant information.
