BY PROVIDING NUTRITIOUS, HOME-DELIVERED MEALS AND INDIVIDUALIZED SUPPORT SERVICES, MOWCM IMPACTS SOCIAL, DETERMINANTS OF HEALTH RESULTING FROM ISOLATION AND POVERTY AND SUPPORTS SUCCESSFUL AGING.
