EARLY CHILDHOOD PROGRAMS: FAMILY LIFELINE'S EARLY CHILDHOOD PROGRAMS PROVIDE INTENSIVE HOME VISITATION SERVICES TO PREGNANT WOMEN AND FAMILIES WITH YOUNG CHILDREN LIVING IN THE GREATER RICHMOND AND PETERSBURG AREAS. SERVICES ARE FREE, VOLUNTARY, AND AVAILABLE UNTIL CHILDREN REACH SCHOOL AGE. WE OFFER THREE HOME VISITING MODELS - HEALTHY FAMILIES, PARENTS AS TEACHERS (PAT), AND CHILDREN'S HEALTH INVOLVING PARENTS (CHIP). AT THE CORE OF ALL THREE MODELS IS THE HOME VISIT. VISITS ARE PROVIDED 2 TO 4 TIMES PER MONTH, LAST APPROXIMATELY AN HOUR, AND TYPICALLY OCCUR IN A FAMILY'S HOME. VISITS ARE PLANNED USING AN EVIDENCE-BASED CURRICULUM AND FOCUS ON FAMILY WELL-BEING, HEALTHY PARENT-CHILD INTERACTIONS, DEVELOPMENT-CENTERED PARENTING, AND PROTECTIVE FACTORS. HOME VISITATION SERVICES INCLUDE ONE-ON-ONE COACHING, EDUCATION, CASE MANAGEMENT, HEALTH SUPERVISION, COUNSELING AND BEHAVIORAL HEALTH SUPPORT, SCREENING AND ASSESSMENTS, SERVICE PLANNING AND GOAL SETTING, AS WELL AS CONNECTIONS TO RESOURCES. IT SHOULD BE NOTED THAT DUE TO THE COVID-19 PANDEMIC, FAMILY LIFELINE'S EARLY CHILDHOOD PROGRAMS QUICKLY PIVOTED TO PROVIDING VIRTUAL VISITS. VIRTUAL VISITS INCLUDE VISITS WITH FAMILIES THROUGH AN INTERACTIVE VIDEO CONFERENCING PLATFORM THAT ALLOWS THERE TO BE TWO-WAY, REAL-TIME, AUDIOVISUAL COMMUNICATION BETWEEN THE HOME VISITOR AND PARENT(S). THESE VISITS ARE DELIVERED USING A DEVICE (LAPTOP, TABLET, OR SMARTPHONE) AND A SECURE VIDEO CONFERENCING PLATFORM. VIRTUAL VISITS ALSO INCLUDE TELECOMMUNICATION VISITS - VISITS COMPLETED VIA PHONE CALLS. PARENTING GROUP CONNECTIONS ARE ALSO OFFERED VIRTUALLY TO FAMILIES. RESEARCH SHOWS THAT EARLY CHILDHOOD EXPERIENCES, BOTH POSITIVE AND NEGATIVE, HAVE A DIRECT IMPACT ON HEALTH, DEVELOPMENT, AND LIFELONG LEARNING. OUR EARLY CHILDHOOD PROGRAMS WORK TO PREVENT ADVERSE CHILDHOOD EXPERIENCES BY INCREASING FAMILY RESILIENCE AND BUILDING ON PROTECTIVE FACTORS. OUR PREMISE IS STRAIGHT-FORWARD, STRENGTHENING FAMILIES PROVIDES A FOUNDATION FOR SUCCESS, TWO GENERATIONS AT A TIME. WE WORK IN PARTNERSHIP WITH FAMILIES TO INCREASE OVERALL WELL-BEING, ACHIEVE STABILITY, AND ENSURE THAT THEIR CHILDREN ARE HEALTHY, SAFE, AND READY TO SUCCEED IN SCHOOL, WORK, AND LIFE. IN FISCAL YEAR 2020-2021, OUR EARLY CHILDHOOD PROGRAMMING EXPERIENCED GREAT SUCCESS. - SERVED 695 FAMILIES/1,091 PARENTS AND CHILDREN (INCLUDES COMMUNITY LIAISON REFERRAL AND OUTREACH) - 100% OF PARTICIPATING FAMILIES REMAINED FREE OF CHILD ABUSE AND NEGLECT - 99% OF PARTICIPATING CHILDREN WERE CONNECTED TO A PRIMARY PROVIDER FOR SICK, ROUTINE, AND PREVENTIVE CARE - 91% OF PARTICIPATING CHILDREN WERE UP-TO-DATE WITH ALL AGE-APPROPRIATE IMMUNIZATIONS - 96% OF PARTICIPATING CHILDREN RECEIVED AGE-APPROPRIATE SCREENINGS FOR POSSIBLE DEVELOPMENTAL DELAYSGeographiesNot indicatedDatesJul 1, 2020 – Jun 30, 2021Source990No causes providedNo populations provided–$1.9MProgram 2 [2021]
LONG-TERM SUPPORT SERVICES: FAMILY LIFELINE'S LONG-TERM SUPPORT SERVICES PROVIDE INTENSIVE, HOME-BASED CARE TO OLDER ADULTS, PEOPLE WITH DISABILITIES, AND FAMILY CAREGIVERS LIVING IN THE METRO RICHMOND AREA. OUR COMPREHENSIVE SERVICES INCLUDE HOME CARE, VISITING VOLUNTEERS, SOCIAL WORK SERVICES/CARE COORDINATION, AND NURSING SUPPORT. ALSO, ALL PROGRAM PARTICIPANTS ARE OFFERED HONORING CHOICES (COUNSELING IN ADVANCE CARE PLANNING) AND NO WRONG DOOR (A VIRTUAL NETWORK DESIGNED TO STREAMLINE ACCESS TO RESOURCES ACROSS THE STATE). RESEARCH SHOWS THAT SOCIAL ISOLATION, FRAILTY, AND CHRONIC HEALTH CONDITIONS INCREASE SHARPLY WITH AGE. ADDITIONALLY, AS THE POPULATION AGES, MORE AND MORE INDIVIDUALS WILL FIND IT DIFFICULT TO CARRY OUT THE EVERYDAY TASKS NECESSARY TO AGE-IN-PLACE AND REMAIN SAFELY AT HOME. FAMILY LIFELINE'S LONG-TERM SUPPORT SERVICES ALLOW INDIVIDUALS AND FAMILIES TO REMAIN SOCIALLY ENGAGED WHILE MAINTAINING THEIR HEALTH, SAFETY, AND DIGNITY. IN FISCAL YEAR 2020-2021, OUR LONG-TERM SUPPORT SERVICES EXPERIENCED GREAT SUCCESS. - PROVIDED NEARLY 23,000 HOURS OF HOME CARE, CARE COORDINATION, AND NURSING SUPPORT - VISITING VOLUNTEERS PROVIDED 1,364 HOURS OF THEIR TIME - HOME CARE PARTICIPANTS SPENT 5,605 ADDITIONAL DAYS IN THEIR OWN HOMES, RATHER THAN A COSTLY FACILITY - BY DELAYING PLACEMENT IN INSTITUTIONAL CARE, THE NET SAVINGS TO THE VIRGINIA MEDICAID PROGRAM TOTALED 881,310 - 93% OF HOME CARE PARTICIPANTS REMAINED FREE OF FALLS AND FALL-RELATED INJURIES WHILE IN OUR CARE - 93% OF SURVEYED PARTICIPANTS REPORTED THAT THEY FEEL LESS LONELY AND MORE SOCIALLY CONNECTED, SINCE STARTING SERVICES - 94% OF SURVEYED PARTICIPANTS REPORTED THAT STAFF AND VOLUNTEERS PROVIDE COMPANIONSHIP AND MEANINGFUL SOCIAL CONTACTGeographiesNot indicatedDatesJul 1, 2020 – Jun 30, 2021Source990No causes providedNo populations provided–$735.5KProgram 3 [2021]
