UP UNTIL JANUARY 31ST, 2019, THE FOLLOWING SERVICE AREAS WERE PART OF COMMUNITY CAREPARTNERS: REHABILITATION HOSPITAL, HOME HEALTH, OUTPATIENT REHABILITATION, HOSPICE, PALLIATIVE CARE, ADULT DAY, PRIVATE DUTY AND ORTHOTICS & PROSTHETICS. THE ORGANIZATION, FORMERLY KNOWN AS COMMUNITY CAREPARTNERS, INC. RECEIVED THE GOVERNOR'S AWARD OF PERFORMANCE EXCELLENCE IN HEALTHCARE. CAREPARTNERS WAS A LOCAL, NOT-FOR-PROFIT HEALTHCARE ORGANIZATION SERVING WESTERN NORTH CAROLINA WITH QUALITY HEALTH SERVICES ACROSS THE FULL CONTINUUM OF POST-ACUTE CARE. ON FEBUARY 1, 2019 CAREPARTNERS CONTRACTED AND BECAME MOUNTAINCARE, INC. MOUNTAINCARE, INC. CONTINUES TO PROVIDE CRITICAL SERVICES THAT WERE PART OF COMMUNITY CAREPARTNERS THAT FULFILL IMPORTANT NEEDS WITHIN OUR WESTERN NORTH CAROLINA COMMUNITIES. BETWEEN THE SERVICES THAT MOUNTIANCARE PROVIDES; ADULT DAY, GRIEF SUPPORT, MUSIC THERAPY AND A HOSPITALITY HOUSE, THEY SERVE ALMOST EVERY AUDIENCE IN THE REGION, FROM CHILDREN TO SENIORS AND FAMILIES TO INDIVIDUALS, ACROSS ALL INCOME LEVELS. WHAT UNITES THE VARIED SERVICES IS THE PASSION OF EVERY STAFF MEMBER AND THEIR COMMITMENT TO PROVIDING THE BEST POSSIBLE CARE TO THE PEOPLE THEY SERVE. EACH OF THE SERVICES HAVE BEEN SERVING THE COMMUNITY FOR 20 PLUS YEARS.THE ADULT DAY PROGRAMS ARE THE ONLY ADULT DAY HEALTH PROGRAMS IN THE COUNTIES THEY SERVE FOR OLDER ADULTS. Our award-winning MountainCare Adult Day Services Centers provide an enjoyable and safe place for aging and impaired adults to stay engaged and active while receiving expert, compassionate care during the day.
