Chapters Health Foundation Inc. is a not for profit organization that was formed to provide financial support to LifePath Hospice, Inc., (LPH) Hernando-Pasco Hospice, Inc. (HPH) Good Shepherd Hospice, Inc. (GSH) and Hospice of Okeechobee (HOK). THE ORGANIZATION ACCEPTS DONATIONS FROM THE PUBLIC. THE NET PROCEEDS OF THESE VARIOUS ACTIVITIES FLOW TO LPH, HPH, GSH, HOK TO SUPPORT ITS END-OF-LIFE HOSPICE SERVICES.
