HIV/AIDS Care Coordination Program
The care coordination program uses a client-centered, collaborative approach to address client care, which helps establish the foundation for a healthy and productive partnership. Care Coordinators support the achievement of client goals by providing tailored care and expertise throughout all phases of living with HIV/AIDS.
