Collaboration Center Foundation has a 5-acre campus that serves families, caregivers and individuals living with Intellectual, Developmental, Physical and Learning Disabilities of all ages through case management and care coordination referral program, support groups, education workshops, early childhood programs and transition services, health and wellness programs, vocational training and employment, life skill training, recreation and family events, resource fairs, individual and family counseling, and telehealth.
