Community Outreach and Support Services
Throughout 2024, our program became a lifeline for many in the community, meeting people where they were and responding to urgent needs with compassion and care. Each mobile outreach encounter5,258 in totalwas more than a number; it was a moment of connection, a chance to offer hope and practical support. In these interactions, we placed 1,799 fentanyl test strips, 1,716 naloxone doses, and 1,050 sterile syringes into the hands of those who needed them most, helping to prevent harm and save lives. Our Gender Affirming & Sexual Wellness Clinic opened its doors to 537 visits, creating a safe space for individuals to receive affirming care without judgment. Support groups welcomed 79 unique participants, offering not just conversation, but communitya place where people could share their stories and find strength together. Education was another cornerstone of our work: 7 overdose response trainings and 5 fentanyl test strip trainings equipped neighbors, friends, and family members with the tools to act in critical moments. Beyond harm reduction, we recognized that stability starts with meeting basic needs. This year, we provided $186,378 in emergency assistance for rent, utilities, and other essentials, helping 20 individuals keep their homes and lights on. Transitional housing support totaled $233,669, giving 14 individuals a safe place to rebuild. And through food distribution, housing security, and stipends, we invested $81,667 to assist 25 individuals, ensuring that no one faced hunger or homelessness alone. Our harm reduction case managers completed 469 visits, walking alongside participants as they navigated complex challenges and connected to care.
