Applications should be submitted in letter form. No standard letter of application is required. A request evaluation will be sent to the applying organization to complete and return. This form emphasizes a description of the charitable organization itself and how the organization is meeting the needs of our communities. A copy of the organization's unexpired IRS letter acknowledging that contributions to the organization are tax deductible must be provided.
