Endeavor Health Integrated Care
Endeavor Health serves as the parent of an integrated health care system, which was created through an affiliation between Edward-Elmhurst Health and Endeavor Health Clinical Operations (formerly NorthShore University HealthSystem). Endeavor Health wholly owns Endeavor Health Physician Partners LLC (EHPP). EHPP is a clinically integrated network and is wholly owned by Endeavor Health. EHPP is the sole owner of NorthShore Physician Partners LLC and Edward-Elmhurst Accountable Care LLC. Endeavor Health wholly owns Endeavor Health Holdco LLC (Holdco). Holdco is the parent entity of GK Medical Management LLC and Imaging Holdings LLC, which are wholly owned. Imaging Holdings LLC wholly owns three imaging centers, including Morton Grove Medical Imaging LLC, Insight Medical Imaging LLC, and Naperville Medical Imaging LLC.
